The information we seek through this questionnaire is in regard to your views on whether Horizons Therapeutic Education Service is the appropriate organisation to open the new Alternative Provision Free School in Somerset.

In order for CLF to open the new SEMH Special School, CLF must enter into a contract (called a Funding Agreement) with the Department for Education.
